Existing imaging reports and original images where available, previous operation notes, the current treatment recommendation, medications, and a short description of daily limitations and goals. There is no need to guess which additional scan to purchase before the hospital reviews the case.

A useful plan explains more than the operation date. It covers inpatient care if needed, mobility support, rehabilitation arrangements, follow-up appointments, companion needs, and when the clinical team expects travel to be appropriate.
